Once your first appointment is scheduled you will receive an email that includes our intake forms along with a verification of benefits form. They are fillable PDF's so that you can type fill them out on your computer. The forms are required to be returned a week before your appointment.

NO SHOW/LATE CANCELLATION ($100 CHARGE)
Our clients are expected to attend each scheduled session on time. A cancelled or delayed appointment delays our work and can negatively impact other clients. Since your appointments involve the reservation of time specifically for you, and out of respect for your therapist and our other clients, a minimum of 24 business hours’ notice is required for rescheduling or canceling an appointment. We appreciate more than 24 business hour notice, when possible, as we can then make that time available to other clients. Therefore, if you should know before the required 24 business hour notice that you will not be able to attend your session, please call the office so that we have time to fill your slot and can put another client in your space that may need it. The more time, the better for the therapist and other clients who are in need and who are currently on a waiting list to be seen.
WHEN THE NO SHOW/LATE CANCELATION FEE IS WAIVED
The only exception to this cancellation policy is in the event of a serious or contagious illness or emergency. We allow one grace for these purposes every six months. Some examples of emergencies are car accidents, deaths in the family or extreme illness. Work issues do not constitute emergencies. In the event of prohibitive weather, we can conduct the session by phone or video.
FREQUENT CANCELLATIONS OR MISSED APPOINTMENT
Frequent cancellations (3 or more in 6 months or 2 consecutive) and/or missed appointments (no show) will result in the termination of therapy. If you have arranged with your therapist to have recurring appointments, the next recurring appointment will stay on the calendar. Therefore, please call the office if you choose to cancel that appointment to avoid a second No Show/Late Cancel charge of $100. A voice mail is sufficient as they are date and time stamped. The recurring appointments will be removed after the second consecutive No Show/Late Cancel. Monday appointments must be cancelled by Friday as no one is in the office Saturday and Sunday to take phone calls and to fill the now empty opening. It remains your sole responsibility to keep track of all scheduled therapy appointments, whether or not you receive the text reminder. It is your responsibility to inform the office if your phone number has changed.
Verification of your insurance and benefits is a courtesy we provide and does not guarantee the insurance company will pay. Ultimately it is up to the client to know their own benefits and work with their insurance company if there is a payment dispute.
ALL CO-PAYS/CO-INSURANCE AND ANY OTHER AMOUNT OWED IS DUE AT TIME OF SERVICE
Our office and billing company provide as a courtesy a quote of benefits for clients and cannot guarantee payment by your insurance company. It is ultimately your responsibility to confirm with your insurance company the cost for services. We highly recommend contacting your insurance company directly to confirm your benefit for services. You will need to give the insurance company your therapist’s name along with two CPT Codes - 90791 and 90837. The insurance company customer service phone number should be on your insurance card.
INSURANCE REPRESENTATIVES OFTEN GIVE THE WRONG BENEIFTS INFORMATION
Our office and our billing management company make every effort to correctly verify a clients insurance benefit. While our verification of benefits is usually accurate there are times the insurance representative will quote the incorrect benefits. Ultimately it is how the claim processes that determines a client’s responsibility. If you believe your amount owed is incorrect it is your responsibility to contact your insurance company to find out why they processed the claim the way they did. We have no control over how an insurance company processes claims and no power to change it. Our quote is a courtesy and cannot guarantee payment by your insurance company. Please contact your insurance company directly to confirm your benefit for services.
